- Payment in full is required at the time of enrollment and prior to the appointment
date.
- The renewal date is each year on the anniversary date of the original enrollment.
- Clients with account balances must be paid in full prior to enrollment in
the program.
- Farm calls are not included in the price of the program. Farm call fees
are determined by location and number of clients to be seen. Farm call fees
and any additional services provided at the time of the Wellness visit are
due and payable on the date of the visit.
- Verbal and/or written credit card authorization is available if preferred
or if the client is not present. Note that for the provision of additional
services, written or verbal authorization is required by the client if they
cannot be present when services are rendered.
- Boosters for the initial vaccine series are not included in the program
& are charged separately and require a separate visit.
- If you do not utilize all the services included in your selected Option,
there is no refund or substitution made on your unused opportunities.
- Prorated refunds will be made in the event of the sale or death of a horse.
- If an Option client does not remain in a “current” financial
status, any remaining Option services (monies) may be utilized to bring the
account current.
- Clients are responsible for setting up appointments for Wellness plan services.
Deer Creek is not responsible for scheduling appointments.
- For scheduling reasons, Wellness appointments must be requested no less
than two weeks in advance.
- To ensure adequate time to evaluate your horse, unscheduled horses will
not be seen during a Wellness visit unless prior notice is given and scheduling
permits.
- A separate appointment is required for dental work under the plan unless
scheduling permits otherwise.
- Pfizer PreventiCare™ or Deer Creek Equine’s deworming program
can be added to all levels of care.
